2021 Medicine Grand Rounds | The Differential Diagnosis of Acute Skin GVHD
City of Hope faculty (MDs and APPs) need to be aware of the multiple cutaneous eruptions that can occur in the post transplant period other than acute Graft versus host disease (GVHD). Rashes occuring after transplant occur with some frequency. The major concern is GVHD in these instances. Hence, many times this is incorrectly diagnosed without adequate attention given to other possible causes. In addition, the clinical morphology and the histopathology of GVHD had significant overlap with other dermatologic entities that can occur in the post-transplant period which requiring different therapy. Ideally practice would be for our clinical teams to be able to appropriately create a differential diagnosis and pursue appropriate workup, noting the limitations of certain testing such as pathologic evaluation.
Physicians and Advanced Practice Providers.
- Identify the differential diagnosis of maculopapular/moribiliform rashes in the post-transplant period.
- Identify the cause of rashes in stem cell transplant recipients.
- Examine the advantages and limitations of a skin biopsy in the immediate post-transplant period.
